Before Your Visit

Consultation checklist.

Bring what lets the consultation start with evidence rather than recollection.

  • Photo ID & insurance card

    Bring current identification and insurance information. Call the office to confirm plan participation before your visit.

  • Imaging

    Bring the actual MRI, CT, or X-ray images when available — not only the written radiology report.

  • Medical records

    Include recent clinic notes, operative reports, pathology, and prior treatment summaries relevant to the concern.

  • Medication list

    List prescriptions, over-the-counter medicines, supplements, allergies, and the doses you currently take.

  • Your questions

    Write down the decisions you want help making, including alternatives, recovery, and what may happen without treatment.

  • A support person

    Consider bringing someone you trust to take notes and help remember the discussion.

  • Records & Imaging

    The office will explain approved transfer options.

    Do not send full medical records through the general website form. After intake, staff can provide instructions for an appropriate transfer method.

  • Insurance & Cost

    Confirm details before care.

    Network participation, referrals, authorizations, and patient responsibility vary. Call the office and your health plan for current information.
